ACCIDENT, INCIDENT REPORTS AND INVESTIGATIONS

OHS Hazard and Incident Reporting Procedure

 
All accidents, incidents or hazardous situations must be reported in accordance with the OHS Hazard and Incident Reporting Procedure. Reports must be made to either the School Safety Officer, the Chairperson of the OHS Committee, or any other School OHS Committee member. Use the following appropriate form:
 
OHS Hazard Report (OHS001)
Used for reporting a hazardous situation where no injury or illness has occurred.
 
Incident and Work Related Illness/Injury Report (OHS002) Describes an accident where a person was injured in terms of incident details, treatment, response, hazard rating and corrective action.
 
Incident Investigation Report (OHS003)
This form should be used for investigation of incidents resulting in 7 days lost time or serious potential risk to health and safety that will require reporting to WorkCover under the Accident Notification Regulation.
Members of the OHSC investigate all accidents and incidents that are reported to it. In the event the incident is notifiable to Workcover, the investigators shall forward the completed Investigation Form to the OHS&WC as soon as possible and within 24 hours.
