All activities within the School must have a documented Risk Assessment performed before they can be undertaken. The Risk Assessment Form must be completed following the UNSW Risk Rating Procedure and submitted for checking by the School Safety Officer before the project or class begins. The original risk assessment is retained by the supervisor and a copy is kept by the Safety Officer in a central register. Risk Assessments must be available to all staff and students involved and all personnel must use the control measures nominated.
 
Risk assessments must be reviewed every five years and modified if necessary. They must also be reviewed and modified if an accident occurs in the meanwhile. A major accident to a student or staff member whom you supervise could lead to extensive fines for you personally. It could also lead to fines for any others with line responsibility and for the university itself.
 
Use the appropriate checklists in assessing risks:
 
RISK REGISTER
 
A Risk Register must be maintained by each supervisor and shall include:
  • A description of all hazards and risks identified
  • The risk rating (low, medium, high)
  • The risk assessment from which the hazard and risk was identified
  • Any associated Safe Work procedures dealing with that hazard or risk.
